You should use the name which is on the GRO index. If you post details of whose cert you want we can search the index for you. List name and approx birth year, also area and county if known.

his name is JOHN CARSEY IN 1841 he was born in 1808 in norfolk ( which is why it sounded carsey to the registrar because)
in 1841 he is living in HECKINGHAM
on the 1841 census it states his birth as
Where born: Norfolk, England
Civil parish: Bunwell
Hundred: Depwade
in 1851 and is today the surnmae is KERSEY
his wife was sophia they wed in 1839 in loddon
he died in 1875 in LODDON norfolk
i found his birth record which says
JOHN KERSEY 1808 VOL 4B PAGE 156 HALES NORFOLK
so do i enter the parish as BUNWELL OR HALES and the surname CARSEY OR KERSEY

If he was born in 1808, you will not be able to puchase a birth certificate as civil registration did not start until Sep 1837.

Hi Shirl the record you found is his DEATH record, not his birth. As Starlight said civil registration didn't begin until 1837.
If you want to send for his death cert. The details you need are:
Deaths Jun 1875
John age 67 Loddon 4b 156
If you do send for the death cert, you enter the details as above, ie place name: Loddon, vol 5b, page 156, qrt Apr-Jun 1875.
